Trauma centers are specialized facilities equipped with the surgical teams, diagnostic technology, and intensive care resources needed to treat the most severe injuries. Unlike a standard emergency room, a designated trauma center is staffed around the clock with trauma surgeons, neurosurgeons, orthopedic specialists, and critical care teams who are ready to act within minutes of a patient’s arrival.

Knowing where the nearest trauma center is located; before an emergency occurs, is important for every California resident. In San Diego County, the trauma system has been recognized as one of the most effective in the country. Since the system’s implementation in 1984, the rate of preventable trauma deaths in the county has dropped from over 21% to less than 1%, according to the San Diego County Emergency Medical Services.

Understanding Trauma Center Levels

Not all trauma centers provide the same level of care. The American College of Surgeons designates trauma centers at different levels based on the resources and capabilities they offer.

Level I Trauma Centers provide the highest level of surgical and critical care. These facilities have 24-hour in-house coverage by trauma surgeons and specialists in areas including neurosurgery, orthopedics, anesthesiology, and critical care. Level I centers also serve as teaching and research hospitals, contributing to the advancement of trauma treatment. They are equipped to handle the most severe and complex injuries, including traumatic brain injuries, spinal cord injuries, and multi-system trauma.

Level II Trauma Centers provide comprehensive trauma care and have 24-hour availability of all essential specialties. While they may not conduct the same volume of trauma research as Level I centers, they provide definitive care for the vast majority of injured patients and often serve as the primary trauma facility in their community.

Level III and Level IV Trauma Centers serve smaller communities and are equipped to provide initial stabilization and assessment. Patients with injuries beyond their capabilities are transferred to a Level I or Level II facility.

San Diego County Trauma Centers

San Diego County’s trauma system includes six designated trauma centers that serve the region’s more than 3.3 million residents. These facilities are geographically distributed to ensure that critically injured patients anywhere in the county can reach definitive care quickly.

UC San Diego Health — Hillcrest (Level I) Located in central San Diego, UCSD’s trauma center is one of the region’s primary Level I facilities. It provides the full spectrum of trauma surgery, neurosurgery, and critical care services and serves as a major academic and research institution.

Scripps Mercy Hospital — San Diego (Level I) Scripps Mercy in the Hillcrest area is a Level I trauma center that has been part of San Diego’s trauma system since its inception in 1984. It treats a high volume of trauma patients annually and offers comprehensive surgical and intensive care resources.

Scripps Memorial Hospital — La Jolla (Level I) Serving the northern coastal communities of San Diego, Scripps La Jolla provides Level I trauma care with around-the-clock surgical teams and diagnostic capabilities.

Sharp Memorial Hospital — San Diego (Level II) Sharp Memorial in the Kearny Mesa area is a Level II trauma center that handles a significant share of the county’s trauma cases. Its modernized emergency department includes a dedicated four-bed trauma resuscitation area with in-department CT scanning.

Palomar Medical Center — Escondido (Level II) Palomar Medical Center serves the inland communities of North San Diego County, including areas near Oceanside and Escondido. As a Level II facility, it provides comprehensive trauma care for patients in the northern and eastern portions of the county.

Rady Children’s Hospital — San Diego (Pediatric Level I) Rady Children’s is the region’s only designated pediatric trauma center, providing Level I trauma care specifically for children. It is staffed with pediatric trauma surgeons, critical care specialists, and child life professionals trained to treat the unique needs of injured children.

Emergency Rooms Across San Diego County

Beyond the county’s six designated trauma centers, dozens of hospital emergency rooms provide acute care for accident injuries that do not meet the threshold for trauma center activation. Major emergency departments in the San Diego area include Sharp Grossmont Hospital in La Mesa (serving the El Cajon and East County communities), Sharp Chula Vista Medical Center (serving Chula Vista and the South Bay), Tri-City Medical Center in Oceanside, and Kaiser Permanente San Diego Medical Center.

When emergency responders arrive at an accident scene, they assess the severity of the patient’s injuries using standardized triage protocols. Patients with the most critical injuries; such as those resulting from catastrophic accidents or high-speed car collisions, are transported directly to the nearest appropriate trauma center, bypassing closer emergency rooms if necessary.
